Difficulties with Ablation for Arrhythmias in Children

چکیده

Radiofrequency ablation procedures in children present unique challenges for the electrophysiologist. At times, obtaining vascular access to reach the heart is a problem. If this first step is accomplished, the small size of the child's heart, arrhythmias relatively unique to the pediatric population, and the presence of congenital heart disease add to the complexity. In this manuscript, a review of commonly encountered problems and suggested solutions based on practice are presented. Precise mapping of the arrhythmogenic substrate, techniques to access excluded portions of the atrium from prior surgery, and the basis for electrophysiology maneuvers important in pediatric ablation are highlighted.

Arrhythmias in Patients With Pulmonary Hypertension; A Narrative Review of the Current Literature

Incidence and prognostic relevance of supraventricular arrhythmias were evaluated in nine studies. These studies investigated patients with different forms of pulmonary hypertension. Supraventricular arrhythmias were more often found in patients with group two pulmonary hypertension. Common findings in these patients were elevated right atrial pressure and diameters and reduced tricuspid annula...

Successful slow pathway ablation in a patient with persistent left superior vena cava.

Persistent left superior vena cava (PLSVC) is a rare vascular variant which can add difficulties to slow pathway (SP) ablation procedures because of the disturbed anatomy of the triangle of Koch. We describe a case of a successful SP ablation using an anatomical approach in a patient with PLSVC.

Emergency diagnosis and management of common pediatric tachyarrhythmia

Tachycardia is common in the pediatric age group. The most commonly seen arrhythmias in children are sinus tachycardia. However, supraventricular tachycardia is the most common tachyarrhythmia that necessitates treatment. Ventricular tachycardia is rare in healthy children but may be associated with sudden cardiac death.  The important risk factors that predispose children for arrhythmias inclu...

How are arrhythmias managed in the paediatric population in Europe? Results of the European Heart Rhythm survey.

The aim of this survey was to provide insight into current practice regarding the management of paediatric arrhythmias in Europe. The survey was based on a questionnaire sent via the Internet to the European Heart Rhythm Association (EHRA) electrophysiology research network centres. The following topics were explored: patient and treatment selection, techniques and equipment, treatment outcomes...

Indications for Electrophysiology Study in children

The advent of electrophysiology (EP) testing revolutionized the care of children with arrhythmia. Precise mechanistic and anatomical diagnosis of arrhythmias became possible. The later development of catheter-based ablation transformed the care of these children by allowing many arrhythmias to be cured during the same procedure. Indications for EP testing vary depending on the age of the child,...
